PLEASE NOTE:
• All proxy forms must be filled out and returned (either by post to AHC – PO Box 214, Altona, 3018 or in person to a current board member) prior to the commencement of the meeting.
• Invitations are hereby requested for positions on the Altona Hockey Club’s Board. Anyone wishing to nominate for a board role must do so prior to the commencement of the meeting either by filling out the attached nomination form or by filling out a form on the day at the venue prior to the Chairman commencing the meeting.
